Nicely formatted printing

Sometimes it's nice to have a physical dictionary or reference for your conlang, so I'd like to add some nice styling for printing the whole dictionary. The dictionary name and description+details would display at the top of the first page, then the words would flow on from there.

I'm thinking multi-column might be a nice touch, and it would be interesting if I could figure out how to predict what pages references might be on to display references by page number instead of with a link. I'm not sure how I would accomplish that yet, but it would be nice to try.
